First, finance or HR log in to the Office365 home page of your company and select Forms.

Then click New Form

Click on the title to modify

Then click Add Question to add a choice question first

Give two options, yes or no, and check the required answer.

Next add text questions

Enter the question as the loan bank. Similarly, do not check the required question.
