Eduardo Rodriguez, the head of the team, said they initially wanted to transplant the eyeball to help James restore his appearance, not to restore James's vision. "if there is some form of vision recovery, that would be great. But our goal is to transplant the eyeball and keep it alive."

Langney Medical Center of New York University explained that whole-eye transplants are different from common corneal transplants in that the eyeball structure is complex, and doctors have to overcome the challenges of nerve connection, immune rejection, and successful blood supply to the retina. Therefore, there is no successful experience of whole-eye transplantation in the past.

The team combined donated eyeballs with donated stem cells, hoping to promote nerve repair between James and the transplanted eyeballs. After the operation, he was unable to open his eyelids and kept wearing an eye patch to protect his eyes.
