CTOnews.com learned that the accident occurred in 2018, when Uber was testing its self-driving car in Tempe, Arizona, USA. Rafaela Vasque worked as a safety guard (driver) in the car test program. Prosecutors described Rafaela Vasque as the vehicle's "eyes and ears", but because of the security officer's "dereliction of duty", he broadcast the TV program "The Voice" on his mobile phone during the autopilot test. As a result, he failed to notice the road conditions and eventually resulted in the death of a 49-year-old woman, Elaine Herzberg.

The National Transportation Safety Board investigated Uber later that year and highlighted a series of safety problems with the company. The committee said that Uber's "inadequate security risk assessment procedures" and excessive trust in security officers, while security officers were not doing their job, eventually led to the tragedy.

Uber stopped the autopilot test immediately after the accident.
